Johna Jaye Kozak, age 31, of Columbus died Tuesday, December 26, 2006 at her home in Columbus. Funeral service are 10:30 a.m. Friday, December 29, 2006 at St. Anthony Catholic Church in Columbus. The Rev. Del Lape will officiate. Visitation is after 5:00 p.m. Thursday at the church. There will be a vigil service held at 7:00 p.m. Thursday evening at the church. Visitation will continue Friday morning 9:30 a.m. until service time at the Church. Burial will be in the All Saints Cemetery. Johna Jaye Kozak was born June 21, 1975 in Lincoln, Nebraska to Roger and Linda (Lott) Berner. She grew up in Lincoln and graduated from Lincoln High School in 1993. She received her Degree in Education from the University of Nebraska-Lincoln in 1997. She married Brian Kozak at St. Mary's Catholic Church in Lincoln on July 13, 2002. She taught at Dawes Middle School in Lincoln. Upon moving to Columbus she substitute taught in the area schools. She was currently on staff at the Columbus Middle School.
